Scope of Services
Concord Health Integrated provides Direct Primary Care (DPC) services, which include but are not limited to general medical consultations, preventative care, chronic disease management, and lifestyle medicine. Additionally, we offer massage therapy, stretch therapy, and functional medicine services, which are not included in the DPC membership fee and must be paid separately.

Direct Primary Care Membership
– DPC membership is a prepaid subscription-based model that grants patients access to primary care services.
– Membership does not constitute insurance and is not a substitute for health insurance coverage.
– Membership fees must be paid on time to maintain access to services.
– Patients may cancel their membership with a [insert notice period] written notice.

Non-Membership Services
– Services such as massage therapy, stretch therapy, and functional medicine guidance require separate payments.

– Concord Health Integrated and its providers are not liable for indirect, incidental, or consequential damages arising from the use of our services.
– We do not guarantee specific outcomes from medical treatment or holistic services.

Termination of Services
– Concord Health Integrated reserves the right to discontinue services to any patient for non-compliance, abusive behavior, or failure to adhere to payment terms.

Governing Law
– These Terms of Use are governed by the laws of the State of Pennsylvania.
– Any disputes shall be resolved through mediation or legal proceedings in the appropriate jurisdiction.
